For The 5th Wave, would I be able to use "Wave" since 'the' is in the title and the next word is a number? It's not the first word/first after 'the'. Or would I have to use 5th and use the spelling not the number?

Yes, I guess it would be arc. My husband has written the book from the diaries of our friends who did a trip from Copenhagen to Istanbul in kayaks. They made this 1½ year trip with their two young children.
