William Sleator is one of my favorite tween/teen Science Fiction writers, I think both of these fit.

Don't think it's what you're looking for, but James Patterson also has a story that has two different story lines. First book is just a duo. Second set is a series with the same characters as the first, from the same original situation, but whole new storyline. First books MA15+, second set are PG13.
When the Wind Blows & The Lake House

The Shadow of Albion 2 part series for this one. There was supposed to be a third book. which wrapped up an over-arcing mystery, but unfortunately, one of the authors died before it was written, so we are left to wonder--whodunit. . .
Dying Bites an FBI agent from our world gets pulled into a parallel world by magic from their side because they need someone with experience dealing/finding serial killers. Interesting idea.
A Connecticut Fashionista in King Arthur's Court this is a comic type series of 3 books.
Witch World This is actually the first book in a really BIG series that spans about 30 or so books. Not all of them deal with the same characters, nor do they all deal with people going to a parallel universe, but there are several characters that do have that happen to them. (This first book is one of the "does" ones.)
Quag Keep gamers wind up in their imagined gaming world--and before you say "done to death" this is, to my knowledge, the first time that idea was used in a book, since it was written back when the only "gaming" was done with paper and pen (think Dungeons and Dragons type games).
The Crossroads of Time 2 books series dealing with an entire culture that travels across worlds as a "normal" part of their civilization.
Ok, ya know--I'm going to just say, check out the works of Andre Norton since she used this idea a LOT. She has another 4 or 5 series/standalones where this happens.
Another Fine Myth humour lives in this series.
Charmed Life a series that deals with traveling to alternate/parallel worlds. This writer (Diana Wynne Jones) also has several series with this idea.
And the Devil Will Drag You Under standalone where the 2 MC's are bebopping around to different parallel worlds. Again, check out this author (Jack L. Chalker) since he has a couple of series with this idea.

Another "Gamers go into game" series is Joel Rosenberg's Guardians of The Flame. It starts with The Sleeping Dragon. I liked the series because it quickly became more than just escapist fantasy. The first book is about trying to get home to help some injured friends, but after that they stay in the fantasy world and try to improve it.
Lord Kalvan of Otherwhen is about a state trooper who is accidentally transported to an alternate world where a tyrannical religious order uses their monopoly on gunpowder to conquer and rule. He becomes quite popular with their current target when he reveals he knows the formula himself.
Them Bones by Howard Waldrop is both an alternate timeline and a time travel book. There are three storylines - an army scout who was part of a time travel mission to fix the past ends up in an alternate North America that Europe never invaded, the rest of his expedition ends up in the Mississippi River valley in the "regular" timeline, long before the Europeans arrived, and an archaeologist in 1930's Mississippi who is desperately trying to make sense of the evidence the second group leaves before the site is destroyed by a flood.
I'm going to agree with Peter about Joel Rosenberg. He does good work and has a well crafted story line.

I'll third this suggestion--great series for the most part--and add Alan Dean Foster's Spellsinger series, about a rock musician who is transported to a reality where most nonhuman animals are sentient.

Split Infinity (first book in the Blue Adept series.) This series deals with 2 identical worlds that over-lap each other.

I'll check it out. Who knows it could be a hidden gem of a book.

I know I read them. Many Waters is my favourite.

And I see you've added Neverwhere, which is one of the best.
