I start typing, see the scene, and explain it. I have tried using outlines and other things like that; they don't work for me, the book kinda goes like it wants to. After I get finished I go back and fix spellings, consistency, and so on, and delete anything that doesn't fit. Mostly. Sometimes my beta readers force me to put stuff back in, I save everything I take out.
