I also edited my OP to reflect some changes; the original information I included for the Kindle edition turned out to be for the UK Kindle edition. I have added the US Kindle edition as well.

As per the new thread about request threads and bump frequency, I'll be bumping this thread on a weekly basis from now on rather than biweekly.
