read this book from my HS library between 2010-2014 so i dont think it would be smutty at all but girl lives in walled city in slum like conditions (dystopian for sure)and is either abducted or escapes to the outside of the wall and is confronted with dangers and mysterious happenings. one mystery i think involves a declining population due to infertility and the FMC observes an autopsy that reveals man actually has ovaries inside. i think there is a bit of romance and i think it is a trilogy/series. please send any suggestions, my memory of the details might be spotty! Could this be Enclave by Ann Aguirre? She lives in sewers/underground town and escapes into aboveground world but it still sucks. They go on scouting missions underground and she falls in love with a boy that is enemies-to-lovers.

Could this be Birthmarked by Caragh M. O'Brien? The story also deals with infertility and she is a midwife. She was and the population is also declining. This sounds similar to the second book in the series. She escapes the city. Look up the description on the first and second book. There was also an autopsy and the same situation happened.
