How much time do you spend revising/editing a book before sending it off? Do you have clean first drafts or do you have to dig back in and fix things right away?

I revise as I write, and consequently I've been told that my "first drafts" loo like third or fourth passes. So I would say I sent me "first drafts" off to my agent or editor, and then I do 3-4 passes with them.
