I read it as a straightforward, accidental death. Perhaps the old woman's greed (too cheap to repair/replace the old stove) killed her!

I thought Maude had done it and that Cormac was going to find her hanging out at her old house nearby when he went there. But she wasn't there and nothing more was said about it. Unlike Tana French, Dervla McTiernan isn't the type who leaves unanswered questions hanging so I would be inclined to think that the death really was accidental.

When Cormac interviewed the old woman, he developed a headache that disappeared when he left and breathed fresh air. I'm sure the author was showing us that there was a problem with the heater. Headache is one of the few signs of carbon monoxide presence.
