For the former, I find that my brain likes to latch onto one aspect of the story or world (big or small) and run with it. It starts out as a mental fan-fiction of sorts, but eventually morphs into its own thing. Songflight, for example, came about after reading Anne McCaffrey's Harper Hall trilogy, but in its final form is very much separate from that world.

Musical inspiration typically comes after the mental fan-fiction stage and is what helps me shape the story, characters, and world into their own things. An instrumental piece may become the barebones of an action sequence, or a single line in a favorite song may birth a character. Another example, Alísa (Songflight's MC) has taken many forms and personalities over the years, but the one constant about her is found in the lyrics to Flyleaf's song "Again"—

With every injustice and deadly fate

And living like it all depends on you
