They are incredibly vague and this seems to be because they operate on a case-by-case basis.

Exactly! Some sort of accountability so you could even try to know what you're looking for to fix. So frustrating that you have to change stuff hoping that it makes it through their checks.
And then there's that scary part of the email where they're like, "IF YOU KEEP SUBMITTING INAPPROPRIATE MATERIAL WE WILL BANHAMMER YOU." That freaked me out so bad. I was like, wait, so not only do I have to keep submitting this until you approve, if I fail, my account will be BANNED?O_O

Yeah - Smashwords will tell you EXACTLY what is wrong and give you multiple stabs at correcting it. Plus, it's always about formatting. They allow all kinds of erotica (barring obviously illegal stuff), marking "adult" content and giving you the option of filtering it from your feed.
I don't see why Amazon can't follow suit.
I've managed to piss off a few people by opposing detailed trigger warnings, but there is such an enormous gulf between what different people find offensive and disturbing that it can never work out for everyone. I always go back to the default position: Warning for graphic sex, violence and/or language is enough.
Let the people self-censor!
