She’s definitely not the first to survive. There are bios of a few others in the Legacy, which Xaviere illustrated for Becoming. In the books, Elena is just the only known one currently alive, and that’s just because so few people–of either sex–survive a bite. Clay’s case was unique. He sought it out, which helped. Being young helped–more resilient, rolling with the changes rather than fighting them. And he was in a place where he was unlikely to be killed by others, until Malcolm found him and Jeremy took him in, which was the final important factor in his survival. All that would have meant he didn’t need any actual werewolf blood.
