SOLVED. Grandfather is a Painter, boy swims in the river near the end of the book [s]

I don't remember how old I was when I read the book but the story was about a boy and his grandfather. His grandfather paints a picture of a boy (i think near the end it was a portrait of himself when he was younger-not sure though) and the strange thing is he "titles" the painting (according to the Main Character's mother, the grandfather never gives a title to his paintings). There's also a girl who seems to mysteriously pop up and helps the boy. The ending was the grandfather dying and the boy swimming in the river because that was what his grandfather wanted to do. I hope the details are enough!
