Hero Gets Heroine Pregnant In The Past, But Not In The Present

I'm posting this topic here as well.

Am I the only one that notices that a hero can get the heroine pregnant in a story about a secret child, but in a story where a hero first meets the heroine and they later have unprotected sex, she doesn't get pregnant. No matter how many times they have unprotected sex.

He just happened to hit her at the right time of the month? Low sperm count. She has an IUD. She's on birth control pills but the author didn't write it in the story. I agree, I'm pretty detailed about info like that, so I tend to tackle it in my stories, but I guess some authors don't feel they need to.
