Sci Fi boy who can create anything from thought

Narrator is a mass murderer for unclear reasons and is protecting the boy from those in power. The boy has animated his toy soldiers and a doll, and they also work to protect him. World is divided into "guilds" or economic groups vs nations and people can use the autonomous global transportation system to commute across continents to work. Transportation system may be sabotaged - there have been several terrible "accidents". Lots of jockeying for power amongst leaders of the groups (China regions, Mitsubishi, Greens, Europe, Technologists, etc.)
