Book about a queen on the run

Around 2018 I read this one fantasy book where a queen had to be on the run and was traveling with one of king's men. Later in the book she discovers she's pregnant, but also has an affair with the guy she's traveling with. At the end of the book she reunites with the king. I think she had some magical powers too. The cover had a queen with a blade/dagger on her hand. Adult fantasy. Was in a mass paperback format. I appreciate any help!
