Amputated hero, used to be a boxer, blames heroine for his condition, a lot of groveling

I don't remember the name of this book. I think the hero was boxer ¿? And his career was really important to him. I remember that hero and heroine were really in love, they got married and had the perfect life.. Hero's family adored heroine and mother in law treated her as a daughter, but one day hero got into an accident ¿? I'm not sure. But the point is that the doctor makes clear to the whole family that if they don't amputate his leg or arm¿?, he will die. But the hero rather die than losing his leg/arm. He also says to the heroine that he will divorce her if she agrees to what the doctor said.
She make her choice and the hero's family let her take the fall and put all the responsabilty of that decision on her only. The hero is amputated and just as he said, he divorces heroine. Years later, we know that he regrets his decision.
The thing is that heroine has this job where she is trying to adopta a kid. Hero and heroine see each other. She doesn't make it easy at all. No to him, not t his family.
He is also trying to adopt someone ¿?
Heroine makes clear to his family that even when things gets better between them, she knows now that they will never be her family, but the hero's family only. And she knows they let her be the bad guy when everyone silently agreed with her about the amputation.
