Please help finding a book on eating disorders.

I do not remember the name, author, or character names. The book is written in a biographical style. I do not know if it was fiction or nonfiction. The book follows a teen girl between middle school to college age. (Probably in high school)
Here are the plot points I remember:

The girl started becoming anorexic, I do not remember the exact trigger, her mother may have played a role in it. I think she knew of other girls who had eating disorders and may have socialized with them.
She started smoking cigarettes to lessen her appetite.
After she developed a habit, she became sick.
She was forced to eat, and she complied, but became bulimic.
I think she had frequent doctor's visits after she was discovered to have eating disorders.
She would feel very sick and would sometimes pass out. I think she may have hallucinated. I think she tried to exercise but felt extremely weak.
Eventually she realized she would die if she didn't eat, and when she tried to eat, she would feel sick and throw up out of habit of being bulimic.
I do not remember how the story resolved.

The book was definitely published before 2010, possibly before 2000 or earlier. I believe I read the book between 2008-2012. Thanks in advance.
