Jack and Jill by Louisa May Alcott has the injury, but Jill's not an orphan (and is quite doted on by her parents IIRC). But if you think you may be conflating several books it's worth a shot. Or maybe there's an overworked servant girl that Jill befriends? Or maybe I'm thinking of Eight Cousins.

Eight Cousins has an orphaned heroine, and she does sprain her ankle quite badly late in the book, but she's also quite well-treated by her family.

Perhaps What Katy Did by Susan Coolidge?
Katy and her brothers and sisters live with their father and aunt as their mother is dead. She loves books and continually gets in trouble until she becomes bedridden after falling off a swing. Eventually she recovers but her illness has changed her and she no longer gets into scrapes but becomes boringly well behaved. I have to say as a child I much preferred naughty Katy!
