Sounds like The Transall Saga by Gary Paulsen
He has to survive on his own until one day he discovers a civilization of “aliens”. The boy eventually realizes that he is on earth and that a disease has caused the people and planet to mutate. He eventually falls in love with one of the girls. In the end, he is transported back to the time he started in and he grows up to find the cure for the disease which caused the future mutation. I think all of those details are correct except maybe the age category. Any help would be much appreciated as this is driving me Crazy haha :)
