Could it have been a Pegasus on the cover? Because it could be City of Stars. It's the second in a series but when the main character goes to sleep holding a winged horse figurine she's transported to an alternate version of Italy.

-She had an ornament that would transport her to another world
-There was a unicorn on the cover of the book, the background was blue
-She goes between the real world and the new world a lot, so she is not stuck
-I believe she must save the fantasy world (could be wrong)
-I also believe she must choose which world to live in, cannot live in both (could be wrong)
I read this book 10-15 years ago, so maybe 2005-2010 and I think it was a cheap book then, not a famous book at all, but I really loved it and I just can't seem to find it again no matter how much googling I seem to do
