I read this book in 2020.
The pregnant man is a surrogate and has been several times which was a deal they made with something like a council so they could be together.
While pregnant he needs or if its the baby that needs closeness.
The baby is the grandchild of another character whose son died, he gives the pregnant man closeness if not sex, can't remember which, when the mate refuses.
The I think dragonshifter has a butler type of person in his home that I think served his father too or something like that and he leaves since the shifter abuses his mate so if I remember correctly.
It does have a happy ending eventually :)
