It’s at least a duology, because I know there was at least a second book. I remember reading it between 2004-2008. It was a fairly new book at the time. I remember the cover had the boy on it and something shiny, maybe his eyes or something he was holding and it I believe had a flap that opened on some versions. It was a unique cover.

The story followed this boy and he ends up in a fantasy world with magical creatures. The magical creatures were sick.

I remember the bad guy (maybe a leprechaun) was this guy selling fake cure all medications/drugs that caused issues to the fantasy creatures. Their was a girl, but I don’t remember if she was from his world or the fantasy world. I remember there was a Sphinx (maybe the girl or she might of been someone else) that he becomes friends with and a library. I remember that they managed to bring a car into the fantasy world(maybe second book). There was a dragon sometime in the story.

I also remember near the end that the evil guy couldn’t handle all the paperwork to sell his fake cure all’s/meds when he went to the boy’s world.

This sounds like The Divide by Elizabeth Kay. Some of the reviews here on Goodreads mention the villain, Snakeweed, selling fake remedies to the creatures.
