Checked this out of my middle school library. It didn't appear brand new and I think was out in paperback by this point.

Set on Earth in the future, a brother and sister team create VR video games. Their games are successful in part because they create a full world - no matter how far you walk you never hit the edge of a map.

Game players begin to fall ill and die and the disease is traced back to and blamed on the brother and sister as it seems to be coming from their game. They go on the run to clear their names/find out who is doing it, ultimately needing to enter the game itself.

They're chased by a police detective whose wife was the first to contract the disease (I think it may have been called Katherine's Disease or something similar). He eventually realizes the siblings are innocent and teams up with them to find the true culprit.

I also very specifically remember that money was referred to as 'credits' and at one point the siblings entered some type of game tournament to earn some while they were on the run.
