SOLVED. Children's Fantasy book. Main character faints & wakes in fantasy world. Puts feet on either side of a river (border) so in "two countries." Mythical characters. Cover: circle in middle, cover opens down the middle (2 flaps). [s]

Elementary level Fantasy Book.
I remember the BOOK COVER being very interesting. I think there was a CIRCLE in the middle and you didn't just open the book like a normal book but you had to "open from both sides", almost as if the circle is the centerpiece and the book opens on either side of it (does this make sense?)
Also, I only remember bits of the book.
The main character puts his/her feet on either side of a river, which makes him/her have the river exactly in the middle of him/her and the character is in "two countries".
The character faints and when waking up is in a fantasy world. From what I remember mystical characters tries to help the character go back WHILE solving something else.

Also, not sure if this is from the same book but I think the character was in his/her's uncle's house and could see magical creatures (after the incident perhaps?) but others could not.

I think I read this before/after 2005...
I have been looking for this book for LITERALLY YEARS, so any help would be very much appreciated.
I never found out the title because I always "re-borrowed" the book by memorizing WHERE it was located in my library and book cover.
