it shows a girl with a broken leg at a coffee shop and she meets a boy, it shows the two different storylines from there either she dates the boy and goes in one direction with her life or doesn't and goes in a completely different direction for her life, I remember the coffee shop was a very popular place where she went a lot, I'm pretty sure she was a big athlete but since her foot was broken she couldn't do practices, I think she also sang or performed there once or twice

Jillian, can you tell us more about the main characters - their ages, high school/college students, jobs, etc.?

This isn't interactive fiction where there are a number of choices with multiple outcomes?

What's the location - country, region, big city/small town?

These don't sound like your book, but they have a similar theme - making a choice and seeing two different outcomes/storylines. I'll mention them just in case you're interested:
