Guy find himself in strange land filled with vampires

I have a recollection of a book about a guy who has I think a car accident and wakes up to find himself in a strange land. The locals are all terrorised by a vampire who lives in a local castle or fort. He helps organise them the defeat the vampire lord and his armies. I remember the sleeve illustration had something that looked like a vampire dog walking on all fours with black candles. Does that ring any bells?
