I use https://www.draft2digital.com/ It set up chapters, embeds table of contents, title, copyright, bio, next read etc. All for free. You can also use it to distribute to just about every outlet apart from amazon and specialist sites such as All Romance.
I use Calibre and dopdf, which acts like a printer, sort of, but 'prints' to a .pdf file. It's free.
Thanks! I've just been trying to figure out the best way to get an epub for my book (with lots of pictures and links) from either a doc or my mobi without doing a ton of footwork (I just need it to give to people for reviews right now)
I had Calibre but found that it failed epub validation and didn't pass Smashword's meatgrinder. What I found extremely useful was using instead of Word, Open Office. Open Office has a plugin called Word2epub and all my epub files pass epub validation. Not a single error.

I am sure there are many such around, but I have found Calibre really simple to use and it is free.
Hope this is useful advice for some :)
