The story involves a young girl and preteen boy running away to the circus. The boy tries his best to take care of the girl but gets restless and eventually leaves her with an older married couple. 

The boy grows to become a successful business man and the girl becomes a popular jewlery designer when their paths cross again. There is an instant attraction but both have their hang ups and baggage.

A major plot point was that the girl is dating her own step brother but does not realize it, while having faint memories of being molested. It's later realized that the stepbrother is the one molested her.

And, were the boy and girl blood siblings or step siblings? If step-siblings who/where were the parents?

I read it about 10 to 15 years ago (sorry I cant be more specific on the timeline). Late 90's early 2000.

I believe it was step siblings. The family was very, very rich and powerful. The step brother was a complete nut job. The one and and only time the girl has sex with the step brother, she stops in the middle and vomits in the bathroom toilet.
