I think it's a fair question. In reality, the fight was only a few blows, but it had a huge impact on Haley. Not every hit someone throws draws blood. It may bruise or break a bone, but it may not draw blood.

Haley's hit drew blood because she hit him in the mouth. That is a spot on the body that bleeds very easily and it bleeds a lot and Matt would not be debilitated from the hit.

Matt hit Haley in the stomach and the back of the head. Those are not spots that will easily draw blood but can knock a person down.

Matt walks away because he's an awful human being. He hit someone he said he cared about and Haley is left lying on the garage floor, not so much because of her injuries, but because of her shock over the ordeal.
