Wow! I am surprised no one else has asked a question yet. Mr. Everett’s writing is always so thought provoking. That said, I would love to know more about “hell for a long-awaited lunch with Mark Twain,” in the authors acknowledgements. What did he mean by that?

Jim "Go to heaven for the climate, hell for the company" is a Mark Twain quote. Everett was playing off that.
Nelda I had wondered about this quote as well, so thanks for the question and the answer. It's funny that Twain felt the most interesting company would be in hell and then Everett would love to lunch with Twain there! Interesting company, for sure!
Lesley I had this question too. Thanks for the answer Jim.
