I think he wants you to assume the worst and to assume that it's all happening with Ratched's knowledge and cooperation - he's priming you to think of her as a villain. Our narrator may be unreliable, but to omit a fact like, "every new patient is raped on the way in the door," is a bridge too far even for him.(less)
