We'll never know for sure, will we? But I don't think Kesey served the story well by merely dropping the note that two aides would be left alone in a closed room with a patient and a jar of Vaseline. I think he wants you to assume the worst and to assume that it's all happening with Ratched's knowledge and cooperation - he's priming you to think of her as a villain. Our narrator may be unreliable, but to omit a fact like, "every new patient is raped on the way in the door," is a bridge too far even for him.

I was wondering this same thing. There was definitely clear abuse, and it seemed to allude to that being of a sexual nature at times, especially with how excited the attendants were about taking patients’ temperatures.
