Are the two accounts set up as the same reviewer type (i.e. Consumer Reviewer, Blogger, etc.)? I know publishes can set things to either request or wish by member type, so maybe that's it?

I can answer this part. Granting wishes is random, see the below help article on Netgalley. The only thing the publisher can choose is how many wishes to grant and which member types to grant them to. But the actual users chosen is completely random.
