Joan Lion sinh ra đã là người hiến tặng. Hình xăm in số 23 trên cổ tay cướp đi của cô quyền công dân, quyền làm người mà lẽ ra cô xứng đáng được nhận. Joan luôn tuân thủ luật lệ của Hệ thống, cố gắng luyện tập, thể hiện để kiếm thêm thu nhập từ những lần hiến tặng. Cô có một mục tiêu lớn, đó là tích cóp đủ tiền để mua quyền công dân cho cô và cha, để thoát khỏi cuộc sống tủi nhục, tối tăm mà cô đang chịu đựng. Những tưởng sự cố gắng ấy sẽ được đền đáp, nhưng rồi người ta phát hiện ra cô có trái tim khỏe mạnh hơn người thường và có ý định bắt cô “nộp thuế tim”.
Hành trình của Joan không đơn thuần là hành trình chạy trốn. Đó còn là hành trình chuyển biến lớn về nhận thức khi cô dám thoát ra khỏi những sự sắp đặt, kìm kẹp của xã hội, tìm lại bản ngã đã đánh đánh mất của mình.
