The title "Unthinkable"correlates to have Omar's visions are so messed up, and disturbing, that no sane person would even imagine up the concepts. It may also relate to how Omar doesn't even control the visions, and how he doesn't even think of the stories, and that they just pop up at random intervals.

The main protagonist, Omar, treats the people close to him, as if he cares for them, and likes being around them. Even though the majority of the time he wants to keep to himself, he still helps out with his mom when she needs help with taking care of his younger brother (5-6), and hanging out with his friend, Jon (10-12).
