Authors argue that many govt. screwups are attributable to 'cultural disconnect' and 'groupthink'; plans made by small groups, in isolation, and often without any meaningful testing, consultation or feedback sought to see if they'll actually work. Poll tax, CSA, tax credits, New Labour ID cards... all eye-wateringly expensive failures in this mold. Sounds spot on.
