Looks like The Historian. It's not YA, but the main girl reads his father's letters, who met Dracula.
Hm, nope, it's not that one. The book seems interesting, though. The other person was also female, I forgot to add.
I'm wondering if this could be Blue Bloods which includes excerpts from a Mayflower period diary. Describing more of the diary might lead to spoilers, so I'll just leave it at that.
Was the girl who wrote the diary the daughter of a doctor? And the vampire the patient of said doctor? Was the story set in England? And the vampire was bringing other vampires in a ship?
The girl from the present was homesick, in the house of her aunt or similar. There was a graveyard near. The aunt/similar brought her a box with old photos or books, or maybe she found it exploring the house. The girl from the past helped in his father's hospital as nurse. They admitted poor people. They had economic difficulties because of this. (view spoiler)
Thanks to Roberto and his read shelf, I found the book!Blood Sinister by Celia Rees. Which, woah, embarrassing. I already had the book shelved in 2009, but not properly, which is probably why I couldn't find it in the mess. Thank you!

I really don't remember the specifics. What I know is that the main character, a girl, was reading someone's diary. Someone who eventually met a vampire. I think it was the same vampire she also met.
All I know is that the book went back and forth between the main character's life and someone else.
It might have also had something to do with someone being a doctor ot working in a psychiatric institution.
