I am not the author of this book - can someone help me correct this?

I am rather upset to find that I am listed as the author of a book called 'Flight of the Wooden Dancer'. This is incorrect. I provided copy-editing services to the author and do not want my name listed on the book entry. Can someone help me to correct this as soon as possible: I would like to remove my name from the listing altogether. I have tried to do this using my GoodReads membership but it did not allow me to do so.
