May be a little soft, but I think Annie Wilkes from Misery by Stephen King is a real nasty B***h! She is the type of monster I really like to read about and watch. She looks normal, acts normal, but in realty is a psycho monster. The 'hobbling' scene still makes me cringe.

The only one that I can think of right now is Randall Flag from The Stand....I would say Jack Torrence from The Shining but he is really more of a victim; Randall Flag is out and out villian to the core!

I guess that would be the Major from Hellsing, if comics and manga are also allowed here. The major is essentially a crazy and nihilistic nazi cyborg bound for massmurder, death and destruction. His "I love war!" speech from the anime adaptation is epic, especially the German version:
