The trouble is which name is the correct one?

Díez or Díaz

Worldcat has both, both are on various covers.

Or is it a matter of having both as this author was several hundred years ago.

The latter, really. From what I can see, some monk likely made a mistake centuries ago and now we don't know which is the real one?

The Spanish Royal Academy of History (can't link, but easy to google) says
