Unfortunately, I don't have a physical copy, only an ebook. In Ukraine, people usually check all these details on the bookseller sites. I believe here (читати уривок) you can find a scan of the colophon page

unfortunately, Yakaboo as a bookseller site can't be used as a source here
and since the book is from 2015, it is not available on publisher's site here

if you have the book in hand, a photo of the front and back cover and title page would work, as long as it shows the ISBN.
