Book for Youth About Troubled Boy Who Travels Back in Time. Read in 1990s.

I read a book sometime in the 1990s (it could have been written in an earlier decade) that was either middle grade or for teens about a boy who found out there was a deadly genetic disorder in his family that he might one day be at risk for-something like Huntington's maybe- I believe one of his parents had been diagnosed with the disease. Anyway, the boy somehow found himself teleported centuries into the past to the time of some plague; I think it was the Black Death but, once again, I'm not 100% certain.

I'd been looking for the book for ages and finally found the title/author but I didn't write it down so I promptly forgot it again. I feel like it was written by a fairly well-known children's/YA author but I've had no luck finding it again. I keep getting results for The Doomsday Book by Connie Willis (also about a Plague and time travel) when I search for it now but that's not the book. The book I read DEFINITELY had a male protagonist and I don't remember it being very science fiction.

I would be so grateful if someone else knew the name of this book or if they could offer any help in finding it (I promise I'll write the title/author down this time if anyone's successful in doing so).

Unfortunately, I didn't recognize any of the books on the time travel list as the one. It's not Children of Winter or The Pit though. There's only one boy who gets sent back in time in the story if I recall correctly, and he's not adopted like the boy in The Pit. Thanks for looking into it though!

Turk, what causes the boy to travel back into the past?

Where does the boy live - location/ country?

I don't remember the location of where the boy was but I feel like his trip into the past was related to some sort of reflective surface, like a mirror or the water in a well.
