SOLVED. YA Fantasy series: Main character can travel between worlds by holding a talisman and closing his eyes. [s]

YA series where the main character (boy) can travel between worlds by the use of a talisman (pretty sure it was a wooden horse).

Anyways, he’s disabled/crippled in the world that he is from, but when he goes to the world with the talisman, he’s whole and well. I just remember at the end he decides to stay in the other world and his body in his “real” world dies. Seriously reminds me of Avatar, but it’s a book. Read this between 2006-2010.

I know there were several books in the series, but they always had different main characters. The idea of traveling with a talisman was the same though. (I think one of the covers had a sparkly blue mask?)
