YA Fantasy. Group of strangers from different worlds united after death to fight evil. Read around 2003-2007.

This was Book I read in Middle school somewhere around 2005 give or take a year.

The general story was about a group of strangers from different worlds each woke up in this same place after dying in specific ways, the main character was a male that had died from drowning during winter. One of the characters was a Centaur.

They were brought together in this same place to train and go fight this one big bad guy, they could all communicate telepathically.

Each of them struggled with a phobia which correlated to how they died (the main character was frightened of water, one character was afraid of tight spaces.

The main character developed a romantic relationship with another member of the group and at the end of the book, they had kids and were able to visit back and forth from this new world and the main character's original world to visit his family.
