YA fiction book. Read sometime in the last 4-5 years. Split perspectives - the grandmother's POV from when she was a teenager, and the teenage granddaughter's POV in current time.

The story focuses on the grandmother's pregnancy out of wedlock from when she was a teenager, and the granddaughter's pregnancy in current time. I believe the grandmother has dementia and is moved into her adult daughter's house, where the adult daughter's son + daughter live as well. The granddaughter goes on a trip while deciding if she wants to keep the baby or not (she ends up keeping the baby). In the grandmother's story, she had became pregnant out of wedlock with a boy of a different race named Sam, and was forced to leave him. She had the baby, but never contacted him. However, throughout the years she had collected newspaper articles about him. Thank you!
