The protagonist's goal was to get to a high enough level in the game that he could get his heart replaced. There was something wrong with his heart (don't recall what), so he needed a new one. He heard about this competition that was taking place in the game, and I think the winners were given whatever they wanted, so he joined in hopes of getting his new heart.
Spoilers: (view spoiler)[He was put on a team with a few other teens his age, and one of them ripped an arm off a monster in the game and got to have a new arm. The boy was super jealous and attacked the monster's corpse ingame, trying to rip its heart out. This did not work. Also, at one point he was given a new heart by the people running the competition, but it was a faulty heart and the people in charge intended to use it to threaten him into compliance. I want to say the four horsemen of the apocalypse showed up at the end, implying a sequel, but I'm not 100% sure that was the same book. (hide spoiler)]
I got this book off of the new-books shelf in the YA section of my library back when I used to read YA, I think between 2013-2017. I'm not certain that I'm recalling the right cover image, but I want to say it was very dark, with a small door open on the left side spilling greenish light across the cover.
