As far as I remember, it was a novel (possibly YA) told from the POVs of both the male protagonist and the female one. The boy was getting abused by someone in his family (either physically or sexually) it might have been his father or his uncle, and, as a coping mechanism, had created a fantasy world/headspace where he "lived in" most of the time. His brother dates a girl who the main protagonist befriends and she tries to help him cope with the trauma. I think she got pregnant, but I'm not sure. The novel was told from both her perspective and the male protagonist's.
I read this around 2012 - 2015.
