In this 1980s (possibly early 1990s) Young Adult contemporary thriller, a teen girl and her father are trapped on an island away from their supplies or maybe possibly shipwrecked. The island that they visit, possibly they have visited it before, because the girl remembers being there before, and I think they have some knowledge of the place. There are criminals or pirates or some type of 'bad guys' on the island, who the girl must avoid.

Father's health is failing , eventually into a diabetic coma because they cannot get back to their supplies to get his insulin - perhaps the bad guys have taken it or are guarding it? Anyway, she is thinking about where to inject him once she gets the insulin, and thinks the stomach is best but she cannot stomach that, so decides on a thigh. I think they are hiding in a cave or some secure spot to avoid the bad guys. Daughter keeps thinking of how to get back to their supplies.

May have been considered a romance because I think I remember a boy with the criminals is friendly and helps her secretly?
