Remove reference asking me to review my own book

I am the author of the above book. Somehow, I inadvertently must have done something on the book's page that now lists me under the reader review section and asks me to write a review. Is there a way to remove this? I didn't mean to do it and I certainly don't wish to review my own book! Thanks.
