I read the book around 2014-2016 but the book is probably older than that.
The book i read it from a teachers library in the back of the class and it didn't have a front cover but the back cover was yellow.

Details: It is about an individual who jumps from person to person fixing their lives and they cant remember the old lives they’ve helped. The individual as well as the people they jumped from appear to be teens. Whenever the old life is good, they move on to another person. the book starts off as they going to a band recital on a school bus and they play an instrument.
