ABANDONED. Y/A, boy in coma travels to an alternate world where he has a special power

I read this book roughly 10-15 years ago. I believe it's a young adult book. I read it in Dutch, but the original could be any language. If I remember correctly a boy falls into a coma and wakes up in an alternate world where he has some special power nobody else has (I think he can "enter" other people to fight them) which he needs to help them defeat someone.

**SPOILERS**

But he slowly starts to wake up in the real world, so his friends break the connection with the real world, he later enters the evil leader and manages to kill him but also ends up dying himself.

Please help me find the title! It drives me insane not knowing. Thank you!
